The Role

  • Lead, coach, and support your team to deliver exceptional care
  • Ensure participant independence, autonomy, and dignity
  • Collaborate with the Interdisciplinary Team (IDT) to coordinate care plans
  • Manage departmental best practices, compliance, and competency
  • Handle participant and vendor concerns with professionalism and empathy
  • Oversee recruitment, training, and performance management of direct reports
  • Support the Center Manager in meetings, initiatives, and daily operations

What We're Looking For

  • Bachelor’s degree or equivalent combination of education and related experience
  • Minimum 2 years’ leadership experience (healthcare leadership preferred)
  • Strong communication, problem-solving, and organizational skills
  • Experience working with the frail, elderly, or long-term care population (preferred)
  • Proficiency in MS Office (Word, Outlook, PowerPoint, Excel)
  • Ability to inspire, coach, and build cohesive teams

Additional Requirements

  • Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
  • Must meet CMS competency standards prior to working independently
  • Comfortable working in varied home environments and all weather conditions
