Job Summary

This is a paraprofessional position that performs a variety of case management duties for Medicare/Commercial Advantage Plans. Job functions include coordinating medical appointments and providing managed care agencies with clinical updates as requested.

Essential Job Functions

  • Review all Medicare Advantage (Managed Care/HMO) admissions and discharges daily.
  • Aid case manager in coordinating follow-up medical appointments.
  • Review all pending medical appointments and follow up with nursing units and transportation department.
  • Provide clinical updates and other documentation requested by Managed Care agencies.
  • Cook up equipment and services for pending discharges.
  • Provide bed management oversight, and other duties as needed.

Education

  • High School Diploma or equivalent.
  • Associate degree preferred.

Experience

  • One-year experience working in a community service agency that provides service to elderly clients preferred.

Licenses/Certifications

  • N/A.

Abilities Required

  • Sensitivity and skill required in working with impaired elders and families by phone.
  • Must be organized, able to work independently and understand confidentiality which is necessary for this program.
  • Must be bi-lingual (English/Spanish).
  • Good communication skills.

  • Participates in departmental activities, meetings and in-services and follows established guidelines.
