Job Summary
This is a paraprofessional position that performs a variety of case management duties for Medicare/Commercial Advantage Plans. Job functions include coordinating medical appointments and providing managed care agencies with clinical updates as requested.
Essential Job Functions
- Review all Medicare Advantage (Managed Care/HMO) admissions and discharges daily.
- Aid case manager in coordinating follow-up medical appointments.
- Review all pending medical appointments and follow up with nursing units and transportation department.
- Provide clinical updates and other documentation requested by Managed Care agencies.
- Cook up equipment and services for pending discharges.
- Provide bed management oversight, and other duties as needed.
Education
- High School Diploma or equivalent.
- Associate degree preferred.
Experience
- One-year experience working in a community service agency that provides service to elderly clients preferred.
Licenses/Certifications
- N/A.
Abilities Required
- Sensitivity and skill required in working with impaired elders and families by phone.
- Must be organized, able to work independently and understand confidentiality which is necessary for this program.
- Must be bi-lingual (English/Spanish).
- Good communication skills.
