About the role
This at-will direct service position is responsible for providing psychosocial, rehabilitation, and recovery-oriented services for adults. The position ensures clients are introduced into the Program in a comfortable, thorough, and timely manner.
Responsibilities
- Completes the initial assessment of clients to confirm eligibility under the target population.
- Supports the development of the Individual Client Plan in collaboration with the member and the treatment team through linkage and handoff at CFT.
- Identifies strengths and challenges of the child and family system.
- Screens for barriers to treatment.
- Identifies the child and family’s natural supports.
- Interacts productively with the treatment team to provide holistic assessment and engagement services that are member-driven and recovery-oriented.
- Completes all required paperwork as assigned promptly.
- Provides prompt intervention in the event of a crisis, both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis.
- Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
- Initiates contact with previous provider, Primary Care Physician, Psychiatric Hospital, and/or collateral resources, to obtain assessment information and provides case management during transition as necessary.
- Ensures that clients discharging from psychiatric hospitals are linked to the agency Psychiatrist within 30 days.
- Ensures clients transferring to services with medication already in place are linked to the agency psychiatrist.
- Develops initial safety plan and monitors safety planning in high-risk cases during assessment period.
- Completes Service Requests and links clients and families to community resources as appropriate.
- Facilitates groups and provides individual and family therapy.
- Supports Family Advocate with engagement of new referrals to the program.
Qualifications
- A typical way of obtaining the knowledge, skills, and abilities outlined above is through graduation from an accredited college or university with a master’s degree in Social Work or related field.
- A minimum of 4 years’ experience as a provider of mental health services.
- Valid registration with the Board of Behavioral Sciences (BBS).
Skills
- Knowledge of psychosocial, rehabilitation, and recovery-oriented services.
- Ability to complete intake Core Assessment, including collateral assessments (CANS, CHQ, MSE).
- Experience in developing Individual Client Plans.
- Proficiency in client engagement and crisis intervention.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.

- Tuesday to Saturday, 2:00 pm - 10:30 pm
- Tuesday to Saturday, 10:00 am - 6:30 pm
