Compensation
The starting pay for this position is $64,000 - $79,000, and is based on non-discriminatory factors such as skills and experience.
Job Responsibilities
- Supports the ASPIRE supervisor and supports coordinators including professional development and training/observation of employee's job duties and functions.
- Manages therapy referrals, psychiatric authorizations, and assists with case discharge documentation.
- Reviews and approves guardianship paperwork, schedules initial treatment planning appointments, and reports data to the team on past due reviews and documentation.
- Provides support to the team members in the supervisor’s absence.
- Coordinates services for a caseload of 35 to children and adults with intellectual and developmental disabilities, ensuring continuity with multiple providers.
- Enhances coping skills and ensures consumers are receiving eligible SSI and Medicaid benefits.
- Completes assessments, diagnoses, and evaluations, and participates in Person-Centered Planning meetings and I.P.O.S. treatment plans.
- Acts as a consumer advocate and completes assessments to identify each person’s needs and desires.
- Maintains and updates clinical knowledge by reading, attending in-service training, and consulting with the clinical supervisor.
- Meets productivity standards for the program and maintains required utilization review scores.
- Provides comprehensive and coordinated care within the framework of the agency’s integrated health care practice model.
- Completes at least 24 hours of in-service training on an annual basis.
Qualifications
- Required: QIDP, Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, or state licensure as a limited or licensed social worker/counselor/marriage and family therapist.
- Preferred: Specialized training in intellectual disability, at least 1 year of experience in training or working with a person who has an intellectual disability, and possession or acquisition of CMHP and QMHP within 1 year of employment.
- Additional: Valid driver’s license, transportation accessible with current auto/vehicle insurance meeting agency policy requirements, proficiency with computers, EHR systems, and Microsoft Office products, experience with HIPAA in health care organizations, ability to work closely with persons with mental or physical limitations, advanced time management, problem-solving, customer service, interpersonal, and conflict resolution skills, and ability to contribute to an inclusive environment.
