Pay

Annual Salary: $70,000

Responsibilities

  • Responsible for the maintenance of program referrals for assigned circuit(s), case assignments, client admissions, Tier assignments or reassignments, and discharge approvals for cases assigned to direct reports. Assume all responsibilities of the Aftercare Navigator when assigned a case.
  • Ensure that all Navigators actively participate in the Transition Conference, Community Reentry Team Meeting, and Exit Conference, and assist with the development of the Transition Plan for Success for each youth served by the Program.
  • Provide oversight to ensure Navigators work closely with the JPO to identify appropriate resources to address the needs of each youth and their family and actively participate in the development of the Youth Empowered Success (YES) Plan for youth on Conditional Release (CR) or Post-Commitment Probation (PCP).
  • Monitor that Navigators initiate services for all referred resources within three (3) working days of the youth's release from the Facility, unless restricted by the resource, and conduct comprehensive reviews of the information available in JJIS.
  • Ensure that Navigators maintain regular contact with youth, their families, and service providers to monitor participation and progress.
  • Establish and maintain professional relationships with DJJ staff, local law enforcement, juvenile court judges, state attorney offices, and public defender offices to support effective program operations.
  • Review and oversee the maintenance of accurate and up-to-date youth case files and case notes by Navigators. Ensure compliance with documentation standards, timely data entry into JJIS, and completion of required tracking and reporting activities.

Requirements

  • A bachelor's degree from an accredited college or university OR an associate degree from an accredited college or university and two years of experience working directly with adolescents in one or more of the following fields: criminal justice (e.g., law enforcement, courts, correction/rehabilitation facilities, or probation/parole), social services (e.g., child protective services, crisis intervention, foster care/group homes, adoption, or mental health/substance abuse treatment), or education.
  • 4+ years of experience working directly with adolescents in a field specified by contract may substitute the education requirement.
  • A minimum of 1 year of supervisory or leadership experience.
  • All personnel providing Direct Care services to youth and families shall be at least twenty-one (21) years of age per DJJ guidelines.
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.
  • Must be able to meet requirements for Eckerd’s Auto Insurance and be able to drive for business purposes. Must maintain a safe driving record and provide annual proof of appropriate auto insurance; must have use of a vehicle capable of transporting three adults, plus driver.
  • This position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.

About Our Program

The purpose of the Aftercare Resource Navigator Program is to increase positive outcomes for youth who are reentering the community from a residential commitment facility by identifying and establishing relationships with resources that address the service needs of youth and families, connecting youth served by the Program and their families with the appropriate resources to address their specific service needs. The Program provides transitional planning services to youth who are pending release from a Facility, including youth who will be supervised on Conditional Release (CR) or Post-Commitment Probation (PCP) upon their release, and Direct Discharge youth, who will be released with no supervision.
