APRN - Wound Care - Per Diem

Responsibilities

• Debridement of wounds by sharp, enzymatic, autolytic, or surgical methods safely completed in the patient’s location, including all associated procedures.
• Application and management of topical wound care and associated wound management products.
• Direction and recommendation of associated services to improve wound healing outcomes, including but not limited to:
  • Labs and radiologic tests
  • Vascular tests
  • Referrals
  • Home health care
  • Vacuum Assisted Closure (VAC) devices
  • Orthotics
• Interpretation of diagnostic tests and recommendation of appropriate follow-up.
• Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing or prevention, within scope of practice, license, and collaborative agreement with the supervising physician.
• Diabetic Foot Ulcer risk assessment for members with diabetes, following ADA guidelines.
• Education of patients on the topical and systemic management of their wounds.
• Education of Primary Care Physicians (PCP), Case Managers (CM), or referring physicians on current wound management practices.
• Provision of regular updates to the PCP, CM, or referring physician on the status of their patient’s systemic conditions associated with wound care.
• Acts as a resource person for decisions, planning, and policies affecting the health of residents.
• Collaborates with staff to identify customer needs and develop plans to meet them.
• Other duties as assigned.

Requirements

• Registered Nurse (RN) preparation and Nurse Practitioner (NP) license required.
• Minimum of 1 year of experience as an NP and 5 years of experience as an RN.
• Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
