About the Role

The Primary Care Office Advanced Practice Provider (Advanced Practice Registered Nurse, APRN) is responsible for providing total care for assigned patients in collaboration with the physician of record and members of the patient care team. The APRN must hold a license in good standing with the State of Florida and will communicate and manage patient care across all care providers, family, and caregivers.

The role ensures timely, patient-centered, evidence-based medicine for patients with acute, chronic, and palliative care needs. The APRN performs patient assessments, creates care plans, and provides interventions, including diagnosing and treating patients, analyzing lab results and x-rays, writing prescriptions, taking patient histories, providing surgical assistance, and ordering home health services, therapies, and durable medical equipment (DME).

The APRN also provides instruction on symptom management and advanced care planning for patients with chronic, life-limiting illnesses, assists in addressing Quality Measures and HCCs, and ensures continuity of care through ongoing communication with the primary care provider. The APRN may collaborate with regional palliative/hospice care agencies to facilitate end-of-life care and services.

Responsibilities

• Provide total care for assigned patients in collaboration with the physician and patient care team.
• Perform patient assessments, develop care plans, and provide acute interventions, chronic care management, and palliative care services within scope of practice.
• Diagnose and decide treatment plans, analyze and interpret x-rays and lab results, write prescriptions, and take patient histories.
• Provide surgical assistance and order home health services, therapies, and DME as indicated.
• Instruct and assist patients with symptom management and advanced care planning for chronic, life-limiting illnesses.
• Address outstanding Quality Measures and HCCs to meet or exceed quality goals.
• Ensure ongoing communication with the primary care provider to coordinate care and consult on complex diagnoses or treatment decisions.
• Collaborate with regional palliative/hospice care agencies to facilitate end-of-life care when appropriate.
• Complete all documentation and billing in a timely manner, per protocol.
• Identify patients who would benefit from skilled nursing or therapy and initiate the appropriate process for qualifying value-based patients.
• Follow CMS guidelines and ensure patients are admitted to qualifying skilled nursing facilities (SNFs).
• Instruct and counsel patients on compliance with therapeutic regimens and health maintenance.
• Refer patients to home care, social services, behavioral health, or other community resources as needed.
• Collaborate with the Program Manager to oversee assigned Medical Assistants, establish workflow processes, and provide ongoing education and training.
• Perform other duties as assigned.

Requirements

• Active and unrestricted license as an Advanced Practice Registered Nurse (APRN) in the State of Florida.
• Strong clinical skills to improve patient care for an aging adult population.
• Excellent interpersonal relationship skills to interpret and communicate healthcare information to patients, families, and the care team.
• Ability to lead in the application of the nursing process to client care, organizational processes, or systems to improve outcomes.
• Proficiency in completing documentation in compliance with Millennium Physician Group (MPG) policy and CMS guidelines.
