About the Role
The Primary Care Office Advanced Practice Provider (Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA)) is responsible for providing total care for assigned patients in collaboration with the physician of record and members of the patient care team. The role involves communicating and managing patient care across all providers, family, and caregivers, ensuring timely, patient-centered, evidence-based medicine for patients with acute, chronic, and palliative care needs.
Responsibilities
- Perform patient assessments, create care plans, and provide interventions as indicated.
- Diagnose and decide treatment plans, analyze and interpret x-rays and lab results, write prescriptions, and take patient histories.
- Provide surgical assistance and order home health, therapies, and durable medical equipment (DME) as needed.
- Offer instruction and assistance on symptom management and advanced care planning for patients with chronic, life-limiting illnesses.
- Address outstanding Quality Measures and HCCs to meet or exceed quality goals.
- Ensure ongoing communication with the Primary Care Provider for continuity and coordination of care; consult with supervising physicians on complex diagnoses and treatments.
- Collaborate with regional Palliative/Hospice Care agencies to facilitate end-of-life care when appropriate.
- Complete all documentation and billing in a timely manner, adhering to protocols.
- Provide services to prevent unnecessary Emergency Department utilization.
- Ensure all pertinent and active medical conditions are documented in the electronic medical record (EMR) in compliance with Millennium Physician Group (MPG) policy and CMS guidelines.
- Identify patients who would benefit from skilled nursing/therapy and initiate the appropriate process for qualifying value-based patients.
- Instruct and counsel patients on compliance with prescribed therapeutic regimens and health maintenance.
- Refer patients to home care, social services, behavioral health, and other community resources as needed.
- Work collaboratively with the Program Manager to oversee assigned Medical Assistants, establish workflows, and provide ongoing education/training.
Qualifications
- Must hold a license in good standing with the State of Florida as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA).
- Completion of a master’s degree in nursing; two or more years of related experience and/or training preferred, or an equivalent combination of education and experience.
