About the role

In this role, you will work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships.

Responsibilities

• Work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care
• Deliver preventive care and chronic condition management
• Support long-term patient relationships

Requirements

• Active Florida APRN or PA-C license
• Primary Care or Family Medicine experience (preferred)
• Strong skills in preventive care and chronic condition management
• Ability to work collaboratively within a team-based care model
