About the Role

Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program. This role offers meaningful work caring for patients with serious illness, strong interdisciplinary support, and the opportunity to practice at the top of license while collaborating with HPM-certified clinicians and supportive program leadership.

Responsibilities

  • Manage initial and follow-up palliative care consultations, prioritizing patients by acuity and collaborating with a dynamic interdisciplinary team.
  • Support complex symptom management, serious illness communication, goals-of-care discussions, hospice evaluations, and end-of-life care planning.
  • Provide initial and follow-up palliative care consultations for inpatient patients.
  • Partner with physicians for collaboration while functioning at the top of the APP scope of practice.
  • Support patients and families through symptom management, hospice evaluations, goals-of-care conversations, and end-of-life planning.
  • Participate in interdisciplinary collaboration with physicians, APPs, hospice liaison support, and palliative counselors.
  • Participate in daily interdisciplinary meetings and regular program leadership/operations meetings.

Schedule

  • Full-time, 40 hours per week.
  • Monday through Friday inpatient consult service with weekend coverage handled through call.
  • Call coverage may be approximately every third week.
  • Daily cadence includes interdisciplinary meetings and regular program leadership/operations meetings.

Pay

Pay Range: $51.05 - $76.60 per hour

Qualifications

  • Nurse Practitioner with appropriate Georgia licensure required or ability to obtain prior to start.
  • Acute Care NP or Primary Care NP certification required, as applicable to licensure and role scope.
  • New graduates will be considered.
  • 2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.
  • Hospice and Palliative Medicine (HPM) certification preferred.
  • Strong skills in symptom management, serious illness communication, and end-of-life care.

  • Primary Program: AHN Palliative Care Program.
  • Care Setting: Inpatient palliative care consult service.
  • Well-established palliative care program with a dedicated 6-bed Palliative Care Unit.
  • Strong interdisciplinary support with a team including a medical director, physicians, APPs, a hospice liaison, and palliative counselors.
  • Collaboration with HPM-certified clinicians and educational partnerships with Mercer University.
