About the Role

Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program. Manage initial and follow-up palliative care consultations, prioritizing patients by acuity and collaborating with a dynamic interdisciplinary team. Support complex symptom management, serious illness communication, goals-of-care discussions, hospice evaluations, and end-of-life care planning.

Care Setting: Inpatient palliative care consult service

Responsibilities

  • Provide initial and follow-up palliative care consultations for inpatient patients.
  • Partner with physicians for collaboration while functioning at the top of the APP scope of practice.
  • Support patients and families through symptom management, hospice evaluations, goals-of-care conversations, and end-of-life planning.
  • Participate in interdisciplinary collaboration with physicians, APPs, hospice liaison support, and palliative counselors.

Procedures are not required for this position.

Team & Support Structure

Program Team: Medical director, physicians, APPs, a hospice liaison, and palliative counselors.

Qualifications

  • Nurse Practitioner with appropriate Georgia licensure required or ability to obtain prior to start.
  • Acute Care NP or Primary Care NP certification required, as applicable to licensure and role scope.
  • New graduates will be considered.
  • 2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.
  • Hospice and Palliative Medicine (HPM) certification preferred.
  • Strong skills in symptom management, serious illness communication, and end-of-life care.
