Core Responsibilities
- Provide comprehensive pre- and post-release reentry planning and coordination for AOT participants with current or recent correctional involvement, including coordination with Maryland correctional facilities, Parole and Probation agents, and the District Court to ensure continuity of AOT Treatment Plan services at the point of release.
- Assist AOT participants in obtaining critical identification and benefits documents required for community reintegration, including Maryland State ID or driver’s license, Social Security card, birth certificate, and proof of citizenship or immigration status, using established state and local document procurement pathways.
- Initiate and facilitate restoration of suspended or terminated public benefits — including Medicaid/HealthChoice, SSI/SSDI, SNAP, and Maryland Energy Assistance — for participants exiting incarceration, coordinating with the AOT Care Coordination Case Manager to prevent gaps in psychiatric medication access and treatment funding.
- Collaborate with the AOT Care Coordination Case Manager and AOT Care Coordination Psychiatrist to incorporate reentry-specific goals and legal compliance requirements into each participant’s AOT Treatment Plan, ensuring court-ordered treatment obligations and community supervision conditions are mutually reinforcing and clearly communicated.
- Serve as the primary liaison between the AOT Care Coordination Team and community supervision officers (Parole and Probation, pretrial services, drug court staff), providing timely treatment compliance updates and facilitating coordinated responses to supervision violations or missed court appearances.
- Conduct individualized reentry risk and needs assessments using validated tools (e.g., LSI-R or equivalent) to identify criminogenic factors, mental health barriers, housing instability, and employment deficits that may impede successful community reintegration, and incorporate findings into the AOT Treatment Plan.
- Connect AOT participants to reentry-specific employment resources including Maryland Workforce Exchange, Second Chance employment programs, ban-the-box employers, certificate-based trade training, and Maryland DORS vocational rehabilitation services tailored to individuals with justice involvement and SPMI.
- Coordinate placement into transitional or permanent housing for participants exiting incarceration who lack stable housing, including referrals to reentry housing programs, faith-based transitional homes, and permanent supportive housing with justice-involved eligibility, working in coordination with the AOT Care Coordination Housing and Employment Specialist.
- Facilitate peer-based reentry support groups for AOT participants with justice involvement, promoting shared experience, accountability, prosocial connection, and recovery-oriented goal setting in a structured group format.
- Provide psychoeducation and skill-building to AOT participants on topics directly relevant to reentry success, including understanding conditions of supervision, navigating the court system, communicating with employers and landlords about justice history, financial literacy, and community re-engagement.
- Document all reentry contacts, referrals, legal coordination activities, participant progress, and compliance updates in the electronic health record (EHR) in a timely, accurate, and HIPAA-compliant manner, consistent with MDH quarterly outcome reporting requirements under Health-General Article §10-6A-12.
- Participate in multidisciplinary AOT Care Coordination Team meetings, clinical supervision, mandatory MDH-required training (HIPAA, MCMRA, 42 CFR Part 2), and Lead4Life quality assurance activities as directed by the AOT Program Director.
- Track and report de-identified reentry outcome data — including benefits restored, IDs obtained, housing placements, employment secured, and recidivism indicators — to support MDH statutory reporting requirements under Health-General Article §10-6A-12 and Lead4Life grant obligations.
- Maintain compliance with all applicable federal and State laws, COMAR regulations, ADA accessibility standards (per §3.2.1.4–3.2.1.5), and Lead4Life policies governing behavioral health services, participant rights, and confidentiality.
- Perform other duties as assigned by the AOT Program Director to support the goals of the AOT Care Coordination Team and Lead4Life’s mission of community-based behavioral health services.
Education and Experience
- Minimum Education: High school diploma or GED required; Associate’s or Bachelor’s degree in Criminal Justice, Social Work, Human Services, Psychology, or a related field strongly preferred
- Certification Required: Must hold, or actively be pursuing, a nationally or state-recognized Certified Reentry Specialist (CRS) credential or equivalent reentry certification (e.g., Maryland Reentry Resource Center certification, NADCP reentry certificate, or ORCA-recognized reentry credential) within 6 months of hire
- Minimum Experience: Minimum 2 years of experience in criminal justice reentry, case management for justice-involved populations, correctional case management, or a closely related human services role; direct experience working with individuals with serious mental illness, co-occurring disorders, or AOT/court-ordered treatment preferred
- Preferred Knowledge: Working knowledge of Maryland’s correctional system, Parole and Probation processes, Maryland District and Circuit Court system, community supervision requirements, and Baltimore City / Anne Arundel / Eastern Shore reentry service landscapes
- Preferred Certifications: Maryland Certified Peer Recovery Specialist (CPRS); LSI-R or validated risk/needs assessment tool certification; Motivational Interviewing (MI) training; Mental Health First Aid
- Preferred: Personal lived experience with the criminal justice system and/or mental health or substance use recovery
- Preferred: Bilingual (English/Spanish)
Additional Requirements
- Valid Maryland driver’s license and insurable driving record required
- Must pass pre-employment drug screening
- Must pass criminal background check (CJIS/FBI) per Maryland law
- Must pass Child Protective Services (CPS) background clearance
- TB test or health screening as required by program
- Current CPR/First Aid certification or willingness to obtain within 30 days of hire
