About the role
As part of a progressive Ambulatory Care pharmacy team at Community Health Network, you will participate in team-based care under approved protocols. You may make medication changes, as appropriate, and manage direct oral anticoagulants and warfarin through our innovative RN-PharmD hybrid anticoagulation service supporting Indianapolis and central Indiana region.
Responsibilities
- Manage direct oral anticoagulants and warfarin through our innovative RN-PharmD hybrid anticoagulation service supporting Indianapolis and central Indiana region.
- Deliver care both in-person and virtually.
- Provide staff education, guideline/policy development and implementation, and participate in performance improvement initiatives and practice advancement.
- Mentor and teach pharmacy students and pharmacy residents.
- Serve as a clinical resource for the healthcare team.
- Advance the level of patient care to achieve the best possible outcomes.
- Provide patient care in the Anticoagulation Clinics and virtually.
- Facilitate and support Anticoagulation Forum Center of Excellence standards.
- Engage in anticoagulation population health projects to impact the lives of patients we serve.
- Monitor for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests.
- Facilitate and support Anticoagulation Forum Center of Excellence standards.
- Have the ability to precept pharmacy students and residents.
Requirements
- Strong knowledge of anticoagulation pharmacotherapy and management of hypercoagulable disease states, atrial fibrillation, venous thromboembolism, peripheral vascular disease, ischemic stroke.
- Strong communication skills and a collaborative approach to patient care.
- Practice at the top of your license utilizing established anticoagulation protocols.
- Meet with patients individually (in-person and remotely) to provide extensive drug and disease state education, in particular peri-procedural management.
- Work closely with physicians and health care teams, while providing direct patient care.
- Work closely with patients to overcome medication adherence issues.
- Able to assist patients in obtaining cost effective anticoagulants with the assistance of medication assistance coordinators and Specialty Pharmacy colleagues.
- Able to answer medication therapy questions as a drug information resource for the other providers and clinic staff.
- Able to perform timely consults for providers and RN collaborators.
Qualifications
- Graduate of an accredited college of pharmacy, Bachelor or PharmD; Doctorate degree.
- Indiana Pharmacist license.
- Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Cardiology or Ambulatory Care focus is strongly encouraged or a minimum of three (3) years working under a drug therapy agreement in a clinical setting.
- Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist, or Certified Anticoagulation Care Provider is encouraged and required within 2 years of accepting the position.
