Ambulatory Oncology Social Worker

Full-time, benefits-eligible position (40 hours per week, Monday - Friday, day shift).

About the Role

Provide psychosocial support, care coordination, and discharge planning services to patients and families. Collaborate with the interdisciplinary team to address social, emotional, and environmental factors that impact patient care and outcomes.

Provide social work services, complex discharge planning, and care coordination to patients and their families. Identify needed community services and refer patients in need to the appropriate services. Collaborate with other members of the multi-disciplinary team to affect positive patient and family outcomes. Communicate with other hospital staff regarding psycho-social issues that impact health condition/status.

Responsibilities

  • Assess patients' psychosocial and discharge planning needs.
  • Develop and implement individualized treatment and care plans.
  • Provide crisis intervention, counseling, and family support.
  • Coordinate internal and external referrals and community resources.
  • Advocate for patients and families to address barriers to care.
  • Collaborate with the healthcare team on discharge planning and transitions of care.
  • Coordinate care conferences as needed.
  • Maintain knowledge of regulations, payer requirements, and reimbursement practices.
  • Document assessments, interventions, and care coordination activities in the EMR.

Requirements

  • Master's Degree in Social Work (MSW) from a CSWE-accredited program

Preferred Qualifications

  • Clinical licensure or provisional licensure
  • Accredited Case Manager Certification (ACM-SW)
  • Healthcare social work experience

Pay

Pay range: $30.70 - $46.05 per hour
