Responsibilities
- Conducts detailed medication assessments
- Supports individualized medication and disease-state management through collaborative care models
- Provides longitudinal follow-up for patients with complex or high-risk medication needs
- Delivers targeted medication education to patients and providers
- Contributes to clinic-level quality and safety initiatives
- Promotes the safe, effective, and cost-conscious use of medications aligned with clinical and operational priorities
Requirements
- PA Pharmacist Licensure
- Board of Pharmaceutical Specialties Certification in area of specialty practice (or Pharmacotherapy if no specialty certification available)
- Bachelor of Science Degree OR Doctor of Pharmacy Degree from an accredited College of Pharmacy
- PGY1 Pharmacy Residency OR 10 years of relevant hospital experience
- PGY2 Pharmacy Residency OR 3 years of relevant hospital experience (Preferred)

- Comprehensive medication management for patients with acute and chronic conditions
- Optimizing therapeutic outcomes, minimizing medication-related risk, and supporting evidence-based prescribing
