Ambulatory Clinical Pharmacy Specialist--full-time, days--Downtown Philadelphia

Responsibilities

  • Conducts detailed medication assessments
  • Supports individualized medication and disease-state management through collaborative care models
  • Provides longitudinal follow-up for patients with complex or high-risk medication needs
  • Delivers targeted medication education to patients and providers
  • Contributes to clinic-level quality and safety initiatives
  • Promotes the safe, effective, and cost-conscious use of medications aligned with clinical and operational priorities

Requirements

  • PA Pharmacist Licensure
  • Board of Pharmaceutical Specialties Certification in area of specialty practice (or Pharmacotherapy if no specialty certification available)
  • Bachelor of Science Degree OR Doctor of Pharmacy Degree from an accredited College of Pharmacy
  • PGY1 Pharmacy Residency OR 10 years of relevant hospital experience
  • PGY2 Pharmacy Residency OR 3 years of relevant hospital experience (Preferred)

  • Comprehensive medication management for patients with acute and chronic conditions
  • Optimizing therapeutic outcomes, minimizing medication-related risk, and supporting evidence-based prescribing
