Ambulatory Clinical Pharmacist- Pulmonology & Asthma

About the role

We're seeking an Ambulatory Clinical Pharmacist specializing in Pulmonology & Asthma to join our dedicated team. As part of our mission to serve, heal, lead, educate, and innovate, we're looking for someone who can provide medication management, monitor and evaluate medication therapy, and educate both peers and patients.

Responsibilities

• Provides pharmacokinetic consultation and evaluates drug medical selection and dosing.
• Evaluates patient response to drug therapy.
• Documents interventions and conducts outcome studies/evaluations.
• Develops pathways, best practices, and guidelines for assigned populations.
• Edits and submits educational materials for publication.
• Mentors residents, students, and new staff.
• Ensures overall patient care across transitions of care.
• Supports the organization during staffing shortages.
• Adapts to the specific needs of the patient population.

Requirements

• Completed Bachelor of Science in Pharmacy
• Completed PGY1 Pharmacy Practice Residency (or equivalent 3 years of related specialty experience)
• Doctor of Pharmacy (PharmD) preferred
• Completed PGY2 Pharmacy Residency preferred
• Current Pharmacy license in the state of practice

Qualifications

• Effective verbal and written communication skills
• Strong leadership and teaching skills

Skills

• Computer skills and dexterity for data entry and patient information retrieval
• Proficiency with Windows-style applications and keyboard
