Sturdy Health Medical Group is seeking a dynamic and patient-centered Ambulatory Clinical Pharmacist Specialist – Endocrinology & Anticoagulation Services to join its growing multidisciplinary care team.

Responsibilities

  • Provide advanced ambulatory pharmacy services through collaborative practice agreements and team-based care models to optimize medication therapy and improve patient outcomes.
  • Function as an embedded member of the Endocrinology, Cardiology, and Primary Care teams and serve as a clinical leader for pharmacist-managed endocrine and anticoagulation services across the medical group.
  • Work in close partnership with physician leadership, population health team, and other healthcare professionals, supporting evidence-based care, enhancing access to clinical pharmacy services, and helping develop innovative ambulatory care programs.
  • Support the development of sustainable pharmacist-led care models that enhance patient access, provider efficiency, and clinical outcomes.

Requirements

  • Active Pharmacist license in the state of Massachusetts
  • Advanced knowledge and understanding of anticoagulation and chronic disease management including diabetes, population health principles, practice models, and the healthcare landscape
  • Experience in quality improvement strategies
  • Demonstrated ability to work on a team and coordinate collaborative efforts
  • Excellent interpersonal, verbal, and written communication skills
  • Strong analytical, problem-solving, and decision-making abilities

Skills

  • Board Certified Ambulatory Care Pharmacist (BCACP)
  • Completion of a PGY2 Ambulatory Care Residency
  • Previous experience managing patients with diabetes, anticoagulation therapy, and other chronic disease states in an ambulatory or population health setting
  • Experience developing and implementing pharmacist-led clinical services or ambulatory care programs
  • Experience precepting pharmacy residents, students, or other healthcare trainees
  • Working knowledge of electronic health record (EHR) systems and population health management tools

Qualifications

  • Doctorate of Pharmacy from an accredited institution required
  • Completion of a PGY1 Pharmacy Practice Residency required, a PGY2 in Ambulatory Care Residency highly recommended

Pay

$109,280.04-$171,079.79
Salary Range Details The pay range displayed on each job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education. This is not inclusive of the value Sturdy Health’s benefits package (if applicable), which includes among other benefits, healthcare/dental/vision and retirement. For annual salaries this is based on full-time employment.
