About the role

The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe, and compassionate nursing care. This role focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, performing clinic-related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes.

The RN Care Coordinator supports assigned physicians by communicating with patients, families, and caregivers, relaying information to the physician, and ensuring understanding of the plan of care. They provide nursing care during clinic sessions, verify completion of orders, and ensure physicians are notified of results.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Serve as a liaison for communication between front desk staff, nursing staff, physicians, and clinic manager.
• Manage care of the patient population by developing expected outcomes, defining resource utilization, and collaborating with providers to set patient goals.
• Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
• Support patient self-management of disease.
• Coordinate continuity of care with patients and families following hospital admission, discharge, and ER visits.
• Manage high-risk patient care, including those with multiple co-morbidities or at risk for hospital readmission, using a registry.
• Develop patient relationships and use assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
• Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits.
• Utilize electronic tools (e.g., EMR) to document assessments, clinical findings, treatment plans, and progress.
• Observe, record, and report patient conditions and reactions to treatments, medications, and diagnostic procedures.
• Maintain and review patient records, charts, and pertinent information; record and follow up on test results.
• Respond to and refer patient treatment-related phone calls; provide medication and treatment instructions.
• Maintain patient confidentiality and comply with practice standards for documentation using EMR.
• Perform the professional, clinical, and technical competencies of the assigned unit or department.

Requirements

• High School Graduate.
• Bachelor’s Degree in Nursing.
• 2 or more years of RN experience.
• Licensed Registered Nurse (LRN) – Multi-State – State Board of Nursing. Must have a current active multistate RN license in good standing in the state of residence. Staff hired prior to 2/1/2024 must obtain a multistate RN license by 4/30/2026.
• Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) certification within 14 days of hire.

Preferred Qualifications

• Previous medical office experience.
