Position Overview
The Ambulatory Clinic Nurse Care Coordinator (RN) provides patient-centered care for individuals with pelvic floor disorders, urinary incontinence, prolapse, and postoperative recovery. Serving as a clinical liaison, they support patients, providers, and departments through education, care planning, triage, and care navigation, balancing hands-on nursing and back-office duties.
Key Responsibilities
- Manage patient flow, triage, and direct care.
- Cookordinate communication among staff, providers, and patients.
- Develop patient outcomes, resource utilization, and support self-management.
- Follow up on care post-discharge, manage high-risk patients, and educate patients/families.
- Utilize EMR for documentation and communicate patient updates.
- Support clinical procedures, verify orders, and ensure compliance with standards.
Qualifications & Skills
- High school diploma and Bachelor of Nursing degree.
- At least 2 years RN experience; preferred prior medical office experience.
- Active multi-state RN license and BLS certification.
- Strong analytical, critical thinking, and communication skills.
- Proficiency in electronic documentation and patient education.
