About the role

The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe, and compassionate nursing care. This role focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, performing clinic-related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes.

The RN Care Coordinator supports assigned physicians by communicating with patients, families, and caregivers, relaying information to the physician, and ensuring understanding of the plan of care. They provide nursing care during clinic sessions, verify completion of orders, and notify physicians of results.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Serve as a liaison between front desk staff, nursing staff, physicians, and clinic managers.
• Develop expected patient outcomes, define resource utilization, and collaborate with providers to set patient goals.
• Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
• Support patient self-management of disease.
• Coordinate continuity of care following hospital admission, discharge, and ER visits.
• Manage high-risk patient care, including patients with multiple comorbidities or high readmission risk, using a registry.
• Develop patient relationships and use assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
• Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits.
• Document assessments, clinical findings, treatment plans, and progress using electronic medical records (EMR).
• Observe, record, and report patient conditions and reactions to drugs/treatments to physicians.
• Educate patients/families about diagnostic procedures, medications, nutrition, and wellness.
• Maintain and review patient records, charts, and pertinent information.
• Record, post, and follow up on test/examination results.
• Respond to and refer incoming patient treatment-related phone calls.
• Instruct patients and families regarding medications and treatment instructions.
• Maintain patient confidentiality and comply with practice standards for EMR documentation.
• Perform professional, clinical, and technical competencies required for the assigned unit/department.

Requirements

• High school diploma or equivalent.
• Bachelor’s degree in Nursing.
• 2+ years of RN experience.
• Basic Life Support (BLS) certification from the American Heart Association (AHA) or obtained within 14 days of hire.
• Active multi-state Registered Nurse (RN) license in good standing in the state of residence or active single-state RN license in the state of work, with proof of application for a multi-state license.
• Staff hired prior to 2/1/2024 must obtain a multi-state RN license by 3/31/2026.

Preferred Qualifications

• Previous medical office experience.

Skills

• Basic typing and word processing ability.
• Excellent analytical, critical thinking, and problem-solving skills.
