Ambulatory Care Pharmacist – Twin Falls Primary Care - 151877

Description

The St. Luke’s Ambulatory Care Pharmacist will be responsible for managing a fast-paced clinic schedule of 12–20 patient appointments per day. Responsibilities include delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education.

Requirements

• Previous experience with the clinical management of anticoagulation or GLP-1 therapy preferred
• Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
• Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)

Qualifications

Preferred qualifications include:
