Ambulatory Care Clinical Pharmacist - Ambulatory Care Clinical Pharmacist - Remote (Ohio License Required)

Responsibilities

• Provide Comprehensive Medication Management (CMM) with clinical care teams
• Develop medication management action plans for self-management goals
• Collaborate with providers to align clinical interventions with medication goals
• Support clinical team with drug information and member-specific needs
• Conduct education sessions and develop tools for medication management
• Create clinical pathways for medication management and disease states

Qualifications

• PGY1 and PGY-2 ambulatory care pharmacy residency required, or 2 years of experience as a clinical pharmacist in ambulatory care setting preferred
