Responsibilities
- Provide Comprehensive Medication Management (CMM) with clinical care teams
- Develop medication management action plans for member self-management
- Collaborate with internal and external providers to align clinical interventions
- Support clinical team members with drug information and member-specific needs
- Conduct medication management in-services and educational tool development
- Help develop clinical pathways and protocols for medication management
Qualifications
- Active Pharmacist license in at least one U.S. state (MI, IL, OH)
- Eligibility and willingness to obtain Ohio licensure upon hire (company-supported)
- PGY1 pharmacy practice residency
- PGY-2 Ambulatory Care Pharmacy Residency or 2+ years as a clinical pharmacist in ambulatory care
- Additional licensure in Michigan or Illinois preferred
