About the role

This position is responsible for coordinating effective use of the pharmacy benefit by delivering clinical and educational interventions designed to improve pharmaceutical care for patients assigned to Housecall Providers. Pharmaceutical care will focus on patients enrolled in primary care, palliative care, advanced illness care, and hospice care programs and will be primarily focused on caring for geriatric patients.

Responsibilities

  • Critically evaluate drug therapy regimens for patients empaneled at Housecall Providers and assist with developing drug treatment plans.
  • Provide medication therapy management (MTM) services.
  • Utilize technology platforms to document and to meet MTM objectives.
  • Participate in quality improvement programs related to the pharmacy program.
  • Develop and conduct retrospective drug use reviews.
  • Consult with clinicians and pharmacists to resolve pharmacy benefit issues.
  • Serve as a drug information resource to patients and other healthcare professionals.

Requirements

  • Graduate of an accredited pharmacy program
  • Current (or eligible) unrestricted licensure as a pharmacist in the state of Oregon
  • Advanced pharmacy training (PharmD, residency, fellowship, board certification or master’s degree in related discipline) or equivalent experience
  • Minimum 2 years as a clinical pharmacist in pharmaceutical care management in an ambulatory care or other clinical setting

Qualifications

  • Knowledge of primary care, the principles of managed care, pharmacy benefit management, pharmaceutical reimbursement, and pharmaceutical utilization
  • Comprehensive, clinical pharmaceutical knowledge base
  • Understanding of geriatrics and palliative care needs and approaches

  • Ability to critically evaluate clinical pharmaceutical and medical literature and apply principles of evidence-based medicine
  • Highly motivated and able to work independently
  • Excellent organizational, project management, and time-management skills

  • Ability to work in diverse settings, such as residential care homes, private homes, and other areas of the community

  • Ability to see, read, hear, and speak clearly for at least 6 hours/day
  • Ability to stand, walk, sit, and perform repetitive finger and wrist movement for at least 3-6 hours/day
