Job Summary

Easterseals PORT Health, a non-profit service provider in North Carolina & Virginia, seeks compassionate, caring, and dedicated individuals to be an AFL Contractor. The role involves guiding and managing Developmental Disabilities or Mental Health Supports for clients living in their individual homes, support programs, or residential settings in Winston-Salem, NC and surrounding areas.

What You’ll Do

  • Guide Program Supervisor to provide case management to clients and their families in collaboration with Managed Care Organizations (MCOs) to set personal plans for daily living and social needs based on individual/family desires.
  • Work with families to coordinate required services and support needs.
  • Provide and promote personal and program development, personnel management, and overall operational administration, including coordination of ongoing activities to meet established goals and objectives of the program.
  • Develop annual budgets for each assigned cost center and provide effective monitoring and financial management of the program.

Position Summary

This position is responsible for the supervision and monitoring of Alternative Family Living (AFL) contractors and developmental disabilities and mental health supports for individuals receiving services within an assigned program. It involves ensuring best practices, supports based on individual/family outcomes, compliance with state, federal, and agency standards, and direct supervision of AFL contractors and Direct Service Providers.
