Responsibilities
- Trains the Medication Assistants/Technicians and provides oversight to the Medication Program, including monitoring the accuracy of the medication distribution system and ensuring that medications are dispensed per physician orders.
- Serves as the primary point of contact for resident care, ensuring that resident care needs are being met and addressing resident complaints and inquiries in a caring and compassionate manner.
- Makes daily rounds to monitor the care provided and ensure that resident care is performed in keeping with the service plan, and that levels of grooming are appropriate.
- Monitors that resident's food intake is appropriate and that all documentation is being performed per unit routine.
- Performs any treatments requiring a license.
- Assists with admission and discharges, including performing admissions valuation, assuring that medical records are completed and medical orders confirmed.
- Performs assessments of residents when there is a change of conditions and follows-up with the appropriate medical team for intervention and notification of family members.
- Responds to Resident complaints and inquiries in a caring and compassionate manner.
- Takes initiative to follow through until service or problem is completed and resolved.
- Maintains accurate and timely documentation of resident care activities and medication administration.
- Ensures that all medications are dispensed per physician orders.
- Monitors the accuracy of the medication distribution system.
Requirements
- Current First Aid Training and Certification.
- 3-5 years in long term care, home care or related field preferred.
- Current LPN/LVN license required.
- Minimum 2 years’ experience in long term care, home care or related field.
- Acute/skilled nursing community experience preferred.
- Knowledge about dementia highly desirable.
- Excellent written and verbal communication skills.
- Knowledge of aging process and implications of aging in a social setting.
- Must understand health care environment and the medical context for older adults.
- Ability to communicate professionally with residents, families, peers and employees in a multi-cultural environment.
- Attention to detail.
- Working knowledge of computers and typing, sufficient to be successful with computerized Point of Care and other systems.
Qualifications
- Completion of a background check. May also include drug screen, physical, TB and fingerprinting per state/local guidelines.
