About the role
The Health Coordinator supports early childhood education programs to ensure children are up to date on EPSDT requirements, receive timely health follow-ups, and have medical and dental homes.
Responsibilities
- Coaches and supports family services and health staff to improve data reliability and outcomes-focused interventions.
- Oversees Health and Disabilities Advocates to ensure EPSDT requirements are met and families have quality medical and dental homes.
- Guides and supports Health and Disabilities Advocates in completing Individual Care Plans for children with chronic health conditions and follows up to ensure effective management.
- Aids family services and health staff in setting individualized evidence-based goals and making demonstrable changes in family life practices.
- Develops and implements targeted recruitment strategies to enroll children with greatest need, including those eligible for childcare subsidies.
- Coordinates with Center leadership to ensure on-time daily attendance and implement strategies to reduce chronic absence.
- Utilizes audit and evaluation data to track child and family outcomes and refine strategies to enhance family impact.
- Works with program leadership to establish monitoring systems for EPSDT adherence, including following up on expired or coming due preventive health visits and follow-up.
- Develops and aligns program-wide health and wellness campaigns with agency mission, promoting healthy lifestyles and habits.
- Supervises assigned department personnel, including hiring, training, scheduling, evaluating, and ensuring accuracy, timeliness, and completion of all work performed by direct reports.
- Maintains accurate records and ensures compliance with all relevant regulations and policies.
Requirements
- Bachelor’s degree in Public Health, Health Education and Promotion, Family and Child Development or related field required; Master’s Degree or R.N. preferred.
- At least 3 years of experience working with diverse families in low-income communities on health and wellness interventions, including evidence-based strategies to reduce health disparities, with supervisory responsibilities.
- Background and understanding of child preventative health, including EPSDT requirements for children 0-5 years old.
- CPR and First Aid Certifications or ability to obtain within 1 year of employment.
- Physical exam and background checks are required for this position.
- Travel required locally or long-distance up to 10% of the time for work-related meetings and functions.
- Must have a valid driver’s license and/or reliable transportation.

$64,000 annually.
