About the role
The Aftercare Caseworker will provide support to youth and their families from the moment of admission until up to a year after discharge from MercyFirst’s residential programs.
Responsibilities
- Monthly meetings with youth and family
- Weekly meetings with youth to discuss activities, needs, and plans
- Weekly meetings with parents or legal guardians
- Collaborative contacts with important figures in a youth's life
- Home assessments within one week of placement
- Participation in weekly treatment rounds
- Recording and updating notes on youth's activities and functioning
- Writing and updating psychosocial assessments
- Participation in red flag meetings
- Participation in placement agency's status meetings
- Completion of monthly detailed discharge plans
- Identification and provision of community-based support
- Attendance at all court appearances after discharge

Pay
$58,000 - $60,000 per year
Hiring bonus of $1,000 after 500 worked hours
