Aftercare Caseworker II

About the role

The Aftercare Caseworker will provide support to youth and their families from the moment of admission until up to a year after discharge from MercyFirst’s residential programs.

Responsibilities

• Monthly meetings with youth and family
• Weekly meetings with youth to discuss activities, needs, and plans
• Weekly meetings with parents or legal guardians
• Collaborative contacts with important figures in a youth's life
• Home assessments within one week of placement
• Participation in weekly treatment rounds
• Recording and updating notes on youth's activities and functioning
• Writing and updating psychosocial assessments
• Participation in red flag meetings
• Participation in placement agency's status meetings
• Completion of monthly detailed discharge plans
• Identification and provision of community-based support
• Attendance at all court appearances after discharge

Pay

$58,000 - $60,000 per year
Hiring bonus of $1,000 after 500 worked hours
