AFC Registered Nurse (Auburn, MA)

About the role

Nonotuck Resource Associates is seeking a Registered Nurse (RN) to provide comprehensive care, assessments, and collaboration.

Key Responsibilities

• Complete intake and assessments for new referrals and as required.
• Conduct nursing assessments and submit Prior Approval requirements.
• Provide supervision, training, and evaluation of caregivers.
• Develop and maintain detailed Individualized Plans of Care.
• Conduct on-site visits with each member per determined need.
• Maintain health status and general needs of members and caregivers.
• Report changes in health status to member's physician and relevant parties.
• Collaborate and communicate effectively with agency employees, caregivers, families, guardians, and funding sources.
• Document nursing progress notes and fulfill other requirements.
• Plan and implement appropriate discharges or transitions of care.
• Provide ongoing educational information and trainings to members and caregivers.
• Perform supervisory duties as assigned.
• Participate in the on-call system as assigned.
• Drive and be flexible to the demands of the job.

Qualifications

• Current MA Registered Nurse License.
• Two years of recent experience working with the elderly or disabled adults.
• Computer and organizational planning skills.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a team environment.
• Prior community experience with the Department of Developmental Services, Adult Foster Care Services, and HCBS waivers ideal.
• Valid driver's license with an excellent RMV record (subject to RMV background check).
