Position Highlights & Primary Responsibilities
- Will join as a AP Practice Partner working with a patient centered team.
- Team includes a Physician, AP, RN, MA, and a PSS (admin).
- Provide care across the continuum, focusing on value-based care and supported by a robust team of specialists.
- Behavioral health providers are integrated into the primary care teams.

Required Qualifications
- Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA).
- At least 1 year of APC experience in a primary care setting.
- Unlimited Licensure in the state of Massachusetts.
- Active DEA License.
Preferred Qualifications
- Experience working within a Team Care model.
- Fluency in EMR systems & technology.
