Qualifications
- 2‑3 years' experience in the primary care/value‑based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
- Active and unrestrictive Texas APRN license
- DEA license or eligibility to obtain it within 2 months of hire
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value‑based care model
- Excellent documentation and patient relationship‑building skills
- Proficient with EMR systems (eCW a plus!)
