About the Role
Acclaim Multispecialty Group, a partner with JPS Health Network, is seeking a motivated and ambitious advanced practice provider to join our dynamic Jail Diversion team. This role focuses on providing an alternative for law enforcement to address the needs of people experiencing a mental health crisis while reducing arrests for non-violent misdemeanor charges. The position is based at the Stop Six/Walter B. Barbour Health Center, located in the heart of The Magnolia Village and Medical District of Fort Worth—a thriving center of culture, commerce, and community.
Responsibilities
- Coordinate multidisciplinary care for a panel of outpatient clinic patients in a Primary Care-Jail Diversion Center.
- Review patient medical records to diagnose, treat, and provide care management and education to patients and families.
- Make appropriate patient referrals and monitor national health guidelines as well as those established by JPS Health Network and Acclaim Multispecialty Group.
- Provide evidence-based, compassionate care to improve patient outcomes while demonstrating patient-centered behavior.
- Maintain a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, and support staff.
- Support academic, educational, and research endeavors, including attending Performance Improvement Programs and Peer Review committee meetings.
- Partner and communicate effectively to build a positive brand and patient/customer experience for JPS Health Network and Acclaim Multispecialty Group.
- Serve as a resource to the community and perform other job-related duties as assigned.
Requirements
- Comfortable working in a large public/county facility.
- Demonstrated working knowledge in the assigned medical specialty area, including competency in assessment techniques, diagnosis, and treatment processes.
- Strong interpersonal, verbal, and written communication skills to collaborate with healthcare professionals and serve as a community resource.
- Self-directed with initiative to develop and participate in community activities and medical outreach programs.
Qualifications
- Graduate of an accredited Nurse Practitioner program with an active Texas license and current Texas APRN license with ACNP certification OR graduate of an accredited Physician Assistant Program with an active Texas license.
- Current, unrestricted medical license to practice in the State of Texas.
- Unrestricted work authorization to work in the United States.
