Advanced Practice Provider - Retail Clinics - South Region

About the role

Community Health Network is seeking an autonomous, board-certified Family Medicine trained Advanced Practice Provider to join their Retail Clinics in the South Region.

Responsibilities

• Provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine.
• Promote a healthy lifestyle and involve clients in their plan of care with emphasis on patient self-determination and maximizing client knowledge.
• Clinics operate Monday - Friday 8:30 A - 7:00 P; Saturday and Sunday 8:30 A - 5:00 P.
• Every other weekend commitment.
• Closed on Christmas, Thanksgiving and New Year's Day.

Requirements

• Minimum two to five years of family medicine or urgent care experience as an advanced practice provider.
• Firmly required to be licensed and certified in the State of Indiana.

Qualifications

• Strong level of autonomy required for this independent position.

Skills

• Excellent communication skills.
• Ability to work independently and in a team setting.
• Knowledge of Epic and access to UptoDate Guidelines.
