Advanced Practice Provider - Primary Care

About the role

Seeking a Primary Care Family Nurse Practitioner (FNP) to support a Community Health Center dedicated to improving access to care in York County. The role involves providing continuity of care for an existing and growing patient population, managing and supporting an established patient panel, including individuals receiving Medication-Assisted Treatment (MAT), and helping establish care for a vulnerable and underserved patient population in a mission-driven outpatient setting. Services include delivering both in-person and virtual primary care services, supporting flexible access for patients, and collaborating within a supportive care team.

Responsibilities

• Provide continuity of care for an existing and growing patient population.
• Manage and support an established patient panel, including individuals receiving Medication-Assisted Treatment (MAT).
• Help establish care for a vulnerable and underserved patient population in a mission-driven outpatient setting.
• Deliver both in-person and virtual primary care services, supporting flexible access for patients.
• Collaborate within a supportive care team consisting of two CRNPs and one physician.
• Practice in a team-based, integrated primary care environment focused on patient-centered outcomes.

Requirements

Required qualifications include a current license to practice as a Family Nurse Practitioner in Pennsylvania, a Master's degree in Nursing, and at least 1-2 years of clinical experience in primary care settings. Experience with Medication-Assisted Treatment (MAT) is preferred.

Schedule

Monday: 8:00 AM – 4:30 PM
Tuesday: 11:00 AM – 7:00 PM (includes virtual patient care hours from 5:00 PM – 7:00 PM)
Wednesday: 8:00 AM – 5:00 PM
Thursday: 10:00 AM – 2:00 PM
Friday: 8:00 AM – 4:30 PM
