About the role

Sentara Health is seeking a full-time Nurse Practitioner or Physician Assistant to join its Palliative Care team on the campus of Sentara Norfolk General Hospital in Norfolk, VA. The Nurse Practitioner or Physician Assistant provides clinical care, support, and education for the palliative patient population as part of the healthcare team.

Responsibilities

  • Perform palliative care consultations and follow-up visits for hospitalized patients with acute palliative care needs, including symptom management and complex medical decision-making
  • Facilitate family meetings related to current medical decision-making and advance care planning
  • Assist with developing appropriate discharge plans with respect to established goals of care, including close collaboration with Hospice for patients to ensure access and continuity of care
  • Actively participate as a member of an interdisciplinary team to ensure palliative care addresses pain and symptom management; psychosocial/spiritual support; and caregiver support

Schedule

Monday–Friday, 8:30 AM – 5:00 PM
