Advanced Practice Provider - PA or APRN - Primary Care

About the Role

An Advanced Practice Provider (APP) delivers direct primary care, synthesizing evidence-based practice, research, and current clinical knowledge. The APP maintains an independent clinic schedule with the possibility of empanelment and demonstrates advanced medical and nursing expertise, clinical judgment, professionalism, and responsibility for ongoing development.

The APP collaborates with a multidisciplinary team and supervising/collaborating physician(s) to perform history taking, clinical assessments, physical examinations, order entry, documentation, interpretation of diagnostic studies, diagnosis, treatment, and care evaluation for the assigned patient population. On-call duties and weekend responsibilities may be assigned.

Responsibilities

• Conduct Quality Assurance chart reviews (25 charts for new graduates, 10 for established providers).
• Evaluate signs and symptoms, including age-appropriate changes, and assess complex acute, critical, and chronically ill patients.
• Develop and prioritize differential diagnoses, identify comorbidities, and use diagnostic strategies to monitor and sustain physiological function.
• Participate in daily team transition huddles to coordinate patient care.
• Utilize evidence-based practice to prescribe pharmacologic and non-pharmacologic treatments, interpret lab and diagnostic testing, and implement treatment plans.
• Ensure quality of care through consultation, collaboration, continuing education, and peer review.
• Adapt teaching approaches based on patient needs, including physiological and psychological changes, developmental stage, and health literacy.
• Collaborate to meet quality core metrics and documentation standards.

Requirements

• Education:
  • APRN: Master’s degree with an Advanced Practice Nursing focus and National Board Certification.
  • PA: Master’s degree with appropriate National Certification.
• Certifications:
  • APRN: National Board Certification as a CNP, CNS, CNM, or CRNA (matching scope of practice).
  • PA: National Commission on Certification of Physician Assistants (NCCPA).
  • Basic Life Support (BLS) certification.
• Licenses:
  • Current Ohio Board of Nursing license for APRN.
  • Current Ohio License 50RX for PAs (with prescriptive authority).
