Responsibilities
Clinical Care
- Provides assessment, triage, and therapeutic management of patient and family needs throughout the disease process as an integral member of an interdisciplinary team.
- Provides optimal patient care in inpatient, ambulatory, and outpatient settings, from diagnostic through prognostic management.
- Works in collaboration with the attending supervising/collaborating physician(s) in accordance with state law, hospital regulations, hospital policy, protocols, and clinical pathways.
- May provide autonomous, team-based care as appropriate.
- Obtains medical histories, performs physical examinations, orders and reviews diagnostic and therapeutic tests and radiologic results, and formulates and manages patient treatment plans.
- Performs procedures and prescribes treatments and medications in accordance with institutional and state guidelines and within specialty scope and professional competency standards.
- May function as a first or second assistant on operative cases, utilizing fine motor skills and manual dexterity, when within position scope and educational and credentialing requirements are met.
- Responds to emergency situations within YNHHS parameters.
Patient Education & Psychosocial Support
- Educates patients and families on care plans and recommended or available resources, including admitting instructions, treatment plans, actual and potential side effects of treatments and medications, and discharge planning.
- Provides psychosocial support to patients, family members, and significant others in a caring, ethical, and professional manner as part of a multidisciplinary team.
Information Management
- Ensures appropriate documentation in accordance with hospital and regulatory requirements.
- Protects patient information in all forms, including electronic, paper, film, voice, or other media, from unauthorized access, modification, destruction, or disclosure.
Quality Management
- Demonstrates a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care.
- Ensures compliance with patient-related healthcare regulatory requirements, standards, and recommendations.
Professional Development
- Engages in activities and projects that maintain and advance professional competency.
- Serves as a clinical resource for new house staff and may provide service-specific orientation related to clinical education.
- Demonstrates high-reliability organizational behaviors, including reporting safety events and near misses.
- Maintains all employee requirements as determined by hospital policy.
- May serve as a clinical preceptor for NP/PA students and other healthcare professionals and may mentor pre-PA/NP candidates.
- May collaborate on research, quality assurance, and performance improvement initiatives and assist in the development of protocols that support clinical practice and professional growth.
Qualifications
Education
- Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
Experience
- Prefer clinical working experience as an NP/PA within the given specialty/service line, but none required.
- APRNs, PAs, CRNAs, and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.
- Evidence of current competence to practice must be obtained during the Medical Staff appointment process to be eligible.
Licensure
For Physician Assistants:
- Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA).
- Connecticut State Physician Assistant License.
- ACLS/BLS Certified, when indicated for position profile.
- DEA License, when indicated for pediatric position profile.
- PALS, when indicated for neonatal care.
- NRP Certification.
For Nurse Practitioners:
- Connecticut State Registered Nurse License.
- Connecticut State Nurse Practitioner License.
- Board specialty certification, as appropriate for position profile.
- ACLS/BLS Certified, when indicated for position profile.
- DEA License, when indicated for pediatric position profile.
- PALS, when indicated for neonatal care.
- NRP Certification.
Additional licensure, certifications, and/or continuing education credits may be required during employment based on position and area of specialty.
