Advanced Practice Provider (NP/PA) – Skilled Nursing Facility
Serving as the facility’s primary internal medicine provider (SNF-ist)
Serve as the facility’s primary internal medicine provider (SNF-ist)
Manage care for short-term rehab and long-term care residents
Perform comprehensive health assessments, physical exams, and diagnoses
Manage acute and chronic conditions following established protocols
Prescribe medications and treatments as appropriate
Conduct monthly stability visits for long-term residents
Evaluate residents with changes in condition
Participate in Advanced Care Planning
Collaboration and Leadership
Collaborate with interdisciplinary teams, medical directors, and attending physicians
Facilitate referrals to specialists and other healthcare providers
Qualifications
- Required: Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina
- Required: Current DEA registration
- Required: Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
- Preferred: At least 1 year of experience in a Skilled Nursing Facility (SNF) or long-term care setting
- Preferred: Experience caring for geriatric populations
