About the role

The Advanced Practice Provider (NP/PA) Heart Transplant I at Cleveland Clinic’s Heart, Vascular and Thoracic Institute works with post heart transplant patients. They triage patients, manage their care in both inpatient and outpatient settings, and adhere to UNOS and CMS regulations.

Responsibilities

  • Conduct thorough medical histories and physical examinations.
  • Initiate and interpret lab tests and other diagnostic procedures.
  • Prescribe and monitor medications as permitted by license.
  • Provide patient education and answer questions about their condition.
  • Collaborate with the medical team during rounds and consultations.
  • Referral to specialists as needed.
  • Ensure continuity of care and discharge planning.
  • Support professional development of the healthcare team.
  • Participate in quality monitoring and research.
  • May serve as a clinical preceptor and onboard new caregivers.

Requirements

  • Nationally certified Advanced Practice Provider.
  • Experience in post-heart transplant care preferred.
  • Licensed as a Nurse Practitioner or Physician Assistant in applicable states.
  • Current certifications including BLS, ACLS, PALS, or NRP.
  • Credentialing and privileging by the Medical Executive Committee and Governing Body of the applicable hospitals.

Qualifications

  • MSN or Master’s degree in Physician Assistant Studies.
  • Current RN license in applicable states.
  • Advanced practice certification from a nationally accredited organization.
  • Experience in collaborative care and complex patient management.

  • Proficiency in electronic medical record systems.

The pay range for this position is $92,620.00 to $157,480.00 annually. Additional benefits include:
