Requirements
- Nurse Practitioner or Physician Assistant with an active North Carolina State License
- 2 or more years of experience
- Strong desire to become a partner with us
- Commitment to providing exceptional patient care in an inclusive, affirming, and non-judgmental environment
Responsibilities
- Practice mindful medicine and treat patients like people rather than numbers
- Engage in a collaborative environment with onsite lab, behavioral health therapists, and nutritionists
- Utilize innovative, integrated telehealth solutions
- Participate in centralized care coordination, care management, and population health management
