Advanced Practice Provider (NP/PA) – Cardiac Support
Responsibilities
- Conduct patient assessments and cardiac evaluations
- Manage chronic cardiac conditions such as CHF, CAD, arrhythmias, and post-MI care
- Assist with medication management, lifestyle counseling, and patient education
- Collaborate with cardiologists to monitor and adjust treatment plans
- Support post-discharge follow-up and transitional care
Qualifications
- Current Nurse Practitioner or Physician Assistant license in Illinois
- Certification as a Nurse Practitioner or Physician Assistant with cardiology experience
- At least 1 year of experience in cardiology or internal medicine preferred
- ACLS and BLS certification required
