About the role
New York Health is seeking a highly motivated and experienced Advanced Practice Provider to join our Breast Surgery Division.
This APP will serve as the clinical lead of the High Risk Breast Program, with primary responsibility for the identification, evaluation, risk stratification, and longitudinal management of patients at elevated risk for breast cancer.
The position requires expertise in breast cancer risk assessment, including the use of the Tyrer-Cuzick (TC) risk model, genetic counseling coordination, and high-risk screening protocols.
The APP will work collaboratively with breast surgeons, medical oncologists, radiologists, genetic counselors, and other members of the multidisciplinary breast care team to deliver comprehensive, evidence-based care.
Responsibilities
- Lead and manage the High Risk Breast Program at New York Health, including program development, patient outreach, and quality improvement initiatives.
- Perform comprehensive breast cancer risk assessments using validated tools, with primary emphasis on the Tyrer-Cuzick (TC) risk score model.
- Identify patients who meet criteria for enhanced screening, chemoprevention, genetic testing, and/or prophylactic surgical intervention.
- Develop and implement individualized risk-reduction and surveillance plans in accordance with NCCN guidelines.
- Maintain a dedicated high-risk patient registry, track outcomes, and generate program performance reports.
- Order and interpret diagnostic studies including mammography, breast MRI, and genetic testing results.
- Evaluate patients with high TC score risk and come up with a plan.
- Cook up referrals for genetic counseling and facilitate multidisciplinary tumor board discussions.
- Provide compassionate counseling regarding surgical options, including prophylactic mastectomy and reconstruction consultations.
- Serve as a point of contact for high-risk TC patients, ensuring continuity of care and timely follow-up.
Requirements
- Minimum of 1-2 years of APP experience required.
- Breast Surgery, Breast Oncology, or Surgical Oncology experience as RN or APP preferred.
- National board certification (AANP, ANCC, or NCCPA).
- Current BLS certification; ACLS preferred.
- Proficiency with Breast Cancer Risk assessment models, specifically the Tyrer-Cuzick (TC) Score.
- Understanding of NCCN guidelines for breast cancer screening and genetic/familial high-risk assessment.
Qualifications
- Master's degree in Nursing or Master's degree in Physician Assistant.
- Board Certification as NP-C or PA-C.
- Experience in breast cancer risk assessment and management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.

Pay
Exact compensation may vary based on skills, experience, and location.
Base Salary Range: between $130,000- $165,000 annually.
