Advanced Practice Provider
Job Overview
The Advanced Practice Provider is a vital member of our Rapid Response team who works collaboratively with the medical and bedside nursing staff in order to appropriately respond to an acute change in patient status, including an adult medical alert. The advance practice provider performs a comprehensive health assessments and management of patients with acute health problems.
Requirements, Licenses, And Certifications
- 1+ years of APP experience required
- CDS and DEABLS and ACLS
- APN Current New Jersey Registered Nurse license
- Current New Jersey Advanced Practice Nurse License
- NP Certification
- PA PA License
- NCCPA
Essential Functions
- Demonstrates an ability to organize and achieve quality performance in new routine
- Demonstrates effectiveness in coordinating aspects of patient care within a surgical health environment
- Demonstrates strong organizational skills
- Communicates effectively and demonstrates time management
- Effectively prioritizes work assignments and demonstrates effective communication skills
- Implements the policies and procedures of the practice
- Performs assessment and reassessment of the patient according to applicable department standards and identified tools
- Performs other duties as assigned by the Physician
- The ability to compassionately interact with patients across different areas of care
- Recognizes areas for improvement in divisional policies or patient care initiates change educate all involved in change
- Regularly communicates with nursing leadership and staff concerning patient care issues
- Thoroughly explains procedures to patients prior to testing
- Provides appropriate patient teaching as
- Obtain patient histories and perform physical examination
- Writing orders and progress notes as indicated
- Assess and manage common health problems
- Order and evaluate pertinent diagnostic tests including but not limited to: Radiology, laboratories and ECGs
- Prescribes pharmacological agents and non-pharmacological treatments for appropriate medical conditions
- Education patients and/or families regarding disease process, medical intervention and health promotion
- Document patient teaching and patient discharge summaries as needed
- Refers and communicates pertinent abnormalities to primary medical team
- Serves as a resource for nursing and ancillary staff in regards to patient and disease management
- Actively participates in sepsis reduction program
