Penobscot Community Health Care is seeking an experienced Nurse Practitioner or Physician Associate to join the Community Care and Geriatrics (CCG) team.

About the role

The CCG team provides in-home care, direct care, and geriatric services in local nursing homes and other long-term care settings in Greater Bangor. The team includes highly skilled physicians, advanced practice providers, licensed clinical social workers, and nurses dedicated to finding the right health care solutions for patients, caregivers, and their families.

Responsibilities

  • Provide in-home care and direct care in local nursing homes and other long-term care settings.
  • Offer geriatric services to patients in various settings.
  • Show a Certificate of Immunization against Rubeola, Mumps, Rubella, Varicella, Hepatitis B, and Influenza.
  • Participate in a rotating call schedule for PCHC CCG patients, typically once a week and one weekend every other month.
  • Drive to patient homes and PCHC/CCG facilities within a 1-hour commute.

Requirements

  • Two years’ experience in clinical practice as a Nurse Practitioner or Physician Associate, preferably in Community Health, Family Practice, or Adult-Gerontology.
  • A valid driver’s license with an acceptable driving record and/or appropriate driving experience.

Qualifications

  • High level of autonomy and flexibility required.
  • Full-time position, 40 hours per week, generally four 10-hour days.
  • Knowledge of state immunization requirements for healthcare workers.

Skills

  • Strong clinical skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage multiple tasks.
