Advance Practice Provider delivering home-based primary and palliative care.

Responsibilities

• Conduct in-home and telehealth visits for medically complex, home-limited patients.
• Perform comprehensive assessments and manage chronic conditions.
• Coordinate interdisciplinary care to improve patient outcomes and reduce avoidable hospitalizations.
• Collaborate with physicians, nurses, social workers, and care managers to ensure patient-centered care.
