Advanced Practice Provider
About the Role
inFORME healthcare is seeking a clinically adept Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work alongside our Physician leaders in a post-acute hospital setting. This unique position spans dual hospital locations in the Elwood, Kansas area, focusing on managing inpatients with higher acuity. Responsibilities include handling complex patient diagnostics such as trach/vent, complicated wounds, and multiple co-morbidities, as well as managing acute changes in place.
The role involves a 7–8-hour on-site shift schedule, sharing the caseload with a physician daily, and weekend rotations to manage patients and collaborate with Hospitalists. You will also work with corporate leadership to maintain optimal quality metrics, implement proprietary protocols, and achieve clinical outcomes.
Responsibilities
- Manage inpatients with high acuity, including trach/vent, complicated wounds, and multiple co-morbidities.
- Respond to and manage acute changes in patient condition.
- Collaborate daily with physicians to share caseload and ensure continuity of care.
- Participate in weekend rotations to manage patients and work with Hospitalists.
- Assist corporate leadership in maintaining quality metrics and implementing proprietary clinical protocols.
Requirements
- Minimum 3 to 5 years of acute inpatient hospital experience as a Nurse Practitioner or Physician Assistant.
- Ability to work autonomously in a fast-paced, collaborative environment.
- Educational background consistent with licensure and certification requirements for the State of Kansas.
