Advanced Practice Provider
About the Role
inFORME healthcare is seeking a dynamic, clinically adept Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our Physician leaders in a post-acute hospital setting. The role involves managing inpatients with higher acuity in the Madison, WI area, including patients with trach/vent, complicated wounds, and multiple co-morbidities. You must be comfortable managing acute changes in place and working autonomously.
Responsibilities
- Manage inpatients with higher acuity, including trach/vent, complicated wounds, and multiple co-morbidities
- Work a shift schedule of 7 on 7 off, including weekend rotations
- Collaborate with Hospitalists and corporate leadership to maintain quality metrics and implement proprietary protocols
- Assist in achieving clinical outcomes and maintaining program components
Requirements
- Minimum 3 to 5 years of acute inpatient hospital experience as a Nurse Practitioner or Physician Assistant
- Ability to work autonomously
- Educational background consistent with requirements for licensure and certification in the State of Wisconsin
