Advanced Practice Provider (APRN/ PA) - Hospital at Home
Description
The External TGH Hospital at Home is a unique hospital medicine service at Tampa General, facilitating patient transfers to home settings for continued care. Patients receive at least two in-person nursing visits and a daily provider visit, which can be virtual or in-person. Advanced Practice Providers (APPs) screen eligible patients within TGH's inpatient and emergency rooms. Clinical responsibilities include daily patient rounding via telehealth, admissions, discharges, and consultations. APPs make autonomous medical decisions, providing comprehensive services and contributing to quality and process improvement initiatives.
Qualifications
- Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant.
- DEA required or obtained after orientation period.
- Inpatient Hospital care and Internal Medicine preferred.
- Safe driving record required to operate hospital at home vehicles.
- Demonstrates excellent communication, critical thinking, and leadership skills.
- Able to work effectively in a team.
- Commitment to ongoing professional development and process improvement.
Work Conditions
Work is performed primarily at TGH main campus and Corporate Center office. Requires travel between TGH facilities and to patients' homes as needed. Shifts include 4-10 hour shifts (Day) with at least 1 weekend per month and rotating remote after-hours on-call 1-2 times per week (subject to change based on staffing needs).
Requires daily interactions with multidisciplinary TGH team members, Shields at Home team members, physicians, consultants, patients, family members, and occasional external parties (media, government officials, public).
