About the role

Join UChicago Medicine, as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) within the Heart Failure department. This is an Outpatient position.

Responsibilities

  • Manage independent clinic templates and work alongside Cardiology Providers in clinic.
  • Work with complex general cardiology and post-hospital discharge patients.
  • Triage and respond to Epic inbaskets.
  • Respond to refill requests, peer-to-peer provider reviews, diagnostic scheduling, and result communication.
  • Perform administrative duties as assigned.
  • Diagnose and manage a full range of cardiology conditions.

Requirements

  • Nurse Practitioner: Master's of Science in Nursing, current and/or eligibility for State of Illinois RN and APN licensure/registration, certified as a Nurse Practitioner by the American Nurse's Credentialing Center or American Academy of Nurse Practitioners, ability to qualify for Illinois and DEA prescribing numbers, certification in BLS/ACLS.
  • Physician Assistant: Completion of an accredited (Physician Assistant) educational program, certified by the National Commission on the Certification of Physician Assistants (NCCPA), current and/or eligibility for State of Illinois licensure/registration, certification in BLS/ACLS.

Qualifications

  • Flexible with other duties as assigned.
  • Accustomed to diagnosing and managing a full range of cardiology conditions.
  • Comfortable identifying and referring patients appropriately to specialized areas of Electrophysiology, Interventional Cardiology, Advanced Heart Failure.
  • Gathers pertinent information systematically and skillfully from all sources: history, diagnostic tests, comprehensive physical examination, and psycho-social assessments.
  • Differentiates normal and abnormal variations for all body systems.
  • Follows up on abnormal imaging, lab tests, etc.
  • Accurately interprets diagnostic tests/lab results.
  • Analyzes multiple sources of data in making clinical judgments and in determining the effectiveness of a plan of care.
  • Develops problem lists with associated differential diagnosis.
  • Incorporates assessment findings into a plan to direct patient care.
  • Plans and implements appropriate pharmacological therapies.
  • Accurately and appropriately performs routine diagnostic and therapeutic techniques according to established protocols and standardized procedures for nurse practitioners.
  • Initiates other referrals based on patient and family need.
  • Sets appropriate consultation dependent upon patient health care needs.
  • Promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of adolescent, adult, and geriatric patients.
  • Participates in research projects in the practice area.
  • Performs other related duties and initiatives as deemed necessary in the best interests of patient care.

Skills

  • Ability to qualify for Illinois and DEA prescribing numbers.
  • Certification in BLS/ACLS.
