Advanced Practice Provider - 32 to 40 Hours, Days - Hartford Satellite

Location: Satellite at Mount Sinai

Scheduled weekly hours: 32, first shift

About the Role

The Advanced Practice Provider provides patient-centered, culturally sensitive, quality care using evidence-based medicine in accordance with hospital policies and protocols. Works in collaboration with the clinical team to provide patient and family education and support. Adheres to granted clinical privileges and collaborative agreement with the collaborating physicians, care protocols, HFSC policy on APPs, and medical staff bylaws, rules and regulations.

Qualifications

• Master's Degree in Nursing with a defined clinical specialty
• Minimum of five years' experience in related clinical practice with demonstrated leadership and teaching skills
• Current licensure as an Advanced Practice Registered Nurse in the State of Connecticut
• Current certification in a specialty from the American Nursing Association
• Passed the State of Connecticut approved Pharmacology course required in Master's Degree Program
• DEA licensure required for narcotic prescription
• Basic and life support certification and advanced cardiac life support (ACLS training)

Responsibilities

• Performs complete history and physical examinations upon admission, annually including pre-operative assessment
• Provides assessment, triage and therapeutic management of patients’ needs
• Requests appropriate consults as needed
• Orders and/or obtains all appropriate laboratory, radiological, and specialized tests to evaluate patients
• Implements care plans in collaboration with clinical team
• Consults with patient, family and, as needed, appropriately qualified physician on care plans
• Provides patient/family counseling and education
• Assists during procedures as privileged
• Provides accurate and timely documentation of all clinical interventions in a clear and objective manner
• Participates and identifies opportunities for improvement in patient care and outcome during daily practice, special projects and per review activities
• Performs other duties as assigned as related to the APP role

Physical Demands

• Sitting/working at a desk and functions associated with clinical tasks (reaching, bending, stretching to access drawers, records, shelves)
• Carrying documents, medical records, books, occasionally small equipment, and medical procedures based on clinical specialties (e.g., endoscopic care, intravenous and medication delivery systems, specialty authorizations, wound ostomy and fistula management)
• Fine manipulation required for writing and some patient care techniques
• Visual/perceptive skills to perform tasks
• Functional use of both upper/lower extremities
• Movement related to walking and standing, fine and gross manipulation
• Occasionally requires pushing and pulling of up to 50 pounds, frequent bending, occasional squatting, kneeling, reaching forward and above shoulders, and twisting

• Working knowledge of professional (medical) terms and definitions

• Ability to comprehend lengthy and complex messages or paragraphs and follow multi-stage and three-step commands
• Ability to perform basic arithmetic operations, compute ratios, rates, percentages, draw bar graphs, and handle American monetary units
